District Medical Group to Close Children’s Rehabilitative Services (CRS) Clinic with Focus on Continuity of Care and Family Support
After extensive evaluation, and with deep respect for its mission and community, District Medical Group (DMG) has made the difficult decision to close its Children’s Rehabilitative Services (CRS) Clinic effective Friday, July 18, 2025, after twelve years.
The decision is a result of the changing nature of the healthcare landscape and variables out of DMG’s control. Evolving and ongoing policies that allow greater flexibility in insurance and provider choice have made it more difficult for DMG to sustain a centralized care model while continuing to meet the needs of the most vulnerable. Additionally, as the Valley’s population has grown and become more geographically dispersed, medical practices and healthcare organizations have expanded, providing families with more care options closer to home. Expanded provider choice and geographic expansion of care options has made it even more challenging to sustain the multispecialty integrated care service model.
In an attempt to adapt to the changing landscape, DMG has pursued new services, expansion of therapies, looked for innovative partnerships, and even explored merger opportunities in order to maintain services. While exceptionally difficult, this transition is necessary to ensure the long-term sustainability and impact of the critically important healthcare services DMG provides to over 90,000 Arizonans annually across more than 50 medical and surgical specialties.
“Serving Arizona families through the CRS program has been one of the greatest privileges and honors of my career,” said Kote Chundu, MD, President and Chief Executive Officer. “This decision was incredibly difficult and only made after alternate options for sustaining the clinic were exhausted. We will be working with all of our valued families and health care partners during this time to ensure continuity and continuation of care with the many high-quality options for service across the Valley,” continued Dr. Chundu.
DMG remains steadfast in its mission and is taking deliberate steps to ensure families, patients, and staff are supported throughout this transition period. Below are important dates and information for families receiving services at the CRS clinic:
- The last day patients will be seen is Friday, June 27, 2025
- Clinic will remain open through Friday, July 18, 2025, to assist with transfer of care coordination.
- DMG strongly encourages patients to begin the process of transferring care to another provider or care setting. DMG suggests that they contact their insurance plan or use their provider directory to assist with seeking a new provider.
- Medical records are confidential and will automatically remain with DMG, as the custodian of records. A copy of medical records can be transferred to another doctor, released to the patient, medical practice, or another person designated, only through expressed written permission and authorization.
District Medical Group will continue providing the best care through their partnership to over 90,000 patients served annually across more than 50 medical and surgical specialties, as one of the largest and most respected physician groups in Arizona.
